Letter of Invitation for Visa Purposes
CIO is proud to welcome international participants for this educational activity. Recognizing that travel visas may be necessary and the process time consuming, please be sure to allow enough time for the process. Detailed information is available at www.travel.state.gove/visa.
A letter of invitation for participation may be provided, upon request, as part of the Visa process. This invitation represents no intent by CIO to provide financial support, nor does it serve as guarantee in any respect to the visa application process. Military Physician

Spouse Registration
Spouse registration, accompanied by a registered attendee, is $200 for the duration of the course. This includes access to all educational sessions, opening reception, continental breakfast, lunch, and breaks. Please note that the fee does not include meeting materials or certificate of attendance.
Cancellation Policy
Cancellation requests received in writing on or before December 28, 2012, will be refunded less a $190 administrative fee. Requests received after December 28, 2012, will not be honored.
